MEMBERSHIP AWARD

The State Association with the largest numeric increase since close of last "membership year" and the State Association with the largest percentage increase since the close of the last membership year.

 

Qualifications:
A certificate shall be presented to the state association showing the largest numerical increase in membership since the close of the last membership year.  A certificate shall be presented to the state association showing the largest percentage of increase in membership since the close of the last membership year.

 

Procedure:

A certificate shall be presented to the State Association showing the largest numerical increase in membership since the close of the last membership year, and to the State Association showing the largest percentage of increase in membership since the close of the last membership year.

NEW MEMBER INVOLVEMENT AWARD (MS-Word Form)

The New Member Involvement Award shall be presented to a member who has shown the most outstanding achievement in promoting the objectives of this Association and furthering the profession of dental assisting.

 

Qualifications

1. Shall be an active or Federal Services member of the American Dental Assistants Association. The nominee must have no more than five (5) consecutive years of membership with the American Dental Assistants Association.

2. Shall not have been a previous new member.

3. Shall have excelled in our motto of 'Education, Efficiency, Loyalty and Service', during years of active membership.

4. Shall have participated on local, state, and/or national offices, Federal Services level and/or ADAA Council or ADAA Special Committee.

5. Shall have excelled in services above and beyond expected duties.

6. Shall not have been a previous recipient of this award. Shall not be an employee of ADAA.

 

Procedure  

1. The candidate’s name and qualifications shall be submitted by a member electronically using Microsoft Word v. 98 or later to the email listed on the awards nomination form by 11:59 p.m. (CST) June 30. All electronically submitted documentation must be typed.

LOYAL ASSISTANT AWARD (MS-Word Form)

The Loyal Assistant Award shall be presented to a member with the longest consecutive employment with one employer. 

 

Qualifications

1. This award shall be presented to an active, life or life retired member of the American Dental Assistants Association with the longest record of consecutive employment with one employer.

2. Consecutive employment shall refer to a member actually employed a minimum of 20 hours per week.

3. The recipient must have maintained a minimum of five (5) consecutive years of membership immediately preceding this award.

4. The recipient shall not have been a previous recipient of this award.

5. The recipient does not have to be present at Annual Conference to receive the award.

6. Shall not be an employee of ADAA.

Procedure

1. Applicant shall submit a notarized statement to include the following:

      -Name of applicant

      -Name of applicant's employer

      -Statement from applicant's employer attesting to length of employment

       

      2. Qualifications and notarized statement shall be submitted electronically using Microsoft Word v. 98 or later to the email listed on the awards nomination form by 11:59 p.m. (CST) June 30. All electronically submitted documentation must be typed.

ADAA/SUNSTAR AMERICA'S PRIDE AWARDS (MS-Word Form)

CLINICAL/BUSINESS/EDUCATOR/FEDERAL

 

The ADAA/Sunstar Americas Pride Award shall be presented to a member who believes in loyalty to self as well as the profession of dental assisting.  The five categories of the ADAA/Sunstar Americas Pride Award will be for: Clinical Assistant, Business/Administrative Assistant, Educator, Federal Services Army Member and Federal Services Air Force Member.

 

Qualifications for Clinical Assistant, Business/Administrative Assistant, and Educator Category
1. Shall be an active, life or life retired member of the American Dental Assistants Association.
2. The recipient must have maintained a minimum of five (5) consecutive years of membership immediately preceding this award.
3. Promote and enhance the profession of dental assisting.
4. Shall have participated on local, state, or national offices and/or ADAA Council or ADAA Special Committee. 
5. Shall not have been a previous recipient of this award.
6. Shall not be an employee of ADAA.

 

Qualifications for Federal Services Army and Air Force Members
1. The recipient must have maintained a minimum of twelve (12) months continuous membership in the Federal Services District immediately preceding this award.
2. Promote and enhance the profession of dental assisting.
3. Shall not have been a previous recipient of this award.
4. Shall not be an employee of ADAA.

 

Procedure 
1. The candidate’s name and qualifications shall be submitted electronically using Microsoft Word v. 98 or later to the email listed on the awards nomination form by 11:59 p.m. (CST) June 30. All electronically submitted documentation must be typed.

JOURNAL AWARD

The Journal Award shall be presented to the ADAA member who has submitted the best article for original publication in the ADAA Journal.

 

Qualifications:
1. Shall be an active, life, life retired or Federal Services member in the American Dental Assistants Association.
2. Shall not be an employee of ADAA.

 

Procedure:

Central Office will send electronically to the council members copies of all articles by ADAA members published in the Journal for the previous year.  An engraved plaque shall be presented to the ADAA member who has submitted the best educational article for publication in an Association publication. We are including the Journal Award although it does not draw nominations from the membership, but is of interest to all Journal readers.

ADAA STUDENT ACHIEVEMENT AWARD (MS-Word Form)

Presented to the ADAA Student member who has shown the most outstanding achievement as a dental assisting student. The ADAA Student Achievement Award may be presented annually to one (1) student member at a dental assisting school.

 

Qualifications:
1. Shall be a student member of the American Dental Assistants Association enrolled in a dental assisting program.
2. Shall be at least halfway through the educational program at the time of application.
3. Participation in class activities, student organization(s), and local/state dental assisting organizations, where available.
4. Shall have demonstrated leadership ability through some activity related to dentistry.
5. Shall not have been a previous recipient of this award.
6. Shall not be an employee of ADAA.

 

Procedure:
1. Typed nomination form along with supporting typed documentation to be received electronically using MS Word v. 98 or later at the email listed on the awards nomination form by 11:59 p.m. (CST) March 31. Self nominations are acceptable. It is the responsibility of the individual submitting the nomination to call central office if an email confirmation stating receipt of nomination materials is not received.
2. ADAA Awards Committee will select an award recipient based on student essay and letters of recommendation. Spelling, grammar and composition of essay will be taken into consideration.
3. Applicant shall submit an essay between 750-1000 words describing the activity completed and answering the following question: Upon graduation, how do you plan to impact the dental assisting profession within your state?
4. Application must be accompanied by a letter of recommendation from a dental assisting faculty member and a dental professional. Recommendation letters cannot be from an immediate family member or nominee shall be disqualified. Recommendation letters should name specific qualities/characteristics that set the applicant apart from others with an example or two of how this candidate exhibited those traits; i.e. critical thinking, initiative, organizational skills, leadership, responsibility, integrity, etc. Letters must be submitted electronically and electronic signatures are accepted.
5. One award recipient shall be awarded a certificate and check for $250.
6. Award recipient since the last Annual Conference shall be announced during Annual Conference.

7. Name, school, personal information, and picture will be published in an official ADAA publication as space allows.

JAS/Oral-B Education Scholarship Program (MS-Word Form)

Juliette A. Southard/Oral-B Education Scholarship Program, named for the founder of the American Dental Assistants Association, is underwritten by Oral-B, ADAA and volunteer donations.  Established in 1929, the program awards scholarships to dental assisting students and dental assistants interested in furthering their education in the field of dental assisting.

 

Qualifications:
1. Candidates must be high school graduates; or hold a GED; or be 18 years of age to be eligible for consideration.           
2. Scholarships are available to students who are enrolled in a dental assisting program or applicable courses aimed at furthering a career in dental assisting.  Applicant must be an ADAA member or ADAA student member.           
3. Transcripts of applicant’s current academic record are required, including college level credits and any current dental assisting program credits.  Examples of acceptable proof of grades to include a computer generated print-out signed by the director; or grades on program stationery if signed by the director. One official transcript copy is sufficient. Proof of acceptance into a dental assisting education program or of current employment in dentistry or dental related field must be submitted. Verification of status must be on program letterhead with the director’s signature or another school official. Two letters of reference are required:  one from a dental professional endorsing the applicant’s qualities pertinent to dental assisting, such as personality, organizational skills, ability to meet people, and the ability to accept responsibility; the other from a person of the applicant’s own choice endorsing integrity and conscientiousness with a brief summary of the applicant’s cooperative and leadership skills. Applicant must submit a letter of intent to pursue a long-range career in dental assisting. Applicant must assume the responsibility of determining whether all supporting documents have been received by the American Dental Assistants Association by the deadline date of March 31st of each year. In the event a scholarship recipient withdraws from school before completing the course of study, the scholarship will be regarded as a non-interest bearing loan and is to be repaid to the Juliette A. Southard/Oral-B Education Scholarship Program. Candidates will be considered on the basis of academic achievement, ability and interest in the career of dental assisting. All electronically submitted documentation must be typed. All supporting documentation must be sent electronically. Handwritten applications will not be accepted.
Recipient shall not be an employee of the ADAA.

 

Procedure:
1. Ten Scholarships in the amount of $750.00 each will be awarded (if financially feasible), for dental assistants or dental assisting students achieving an average of 80 or higher in the evaluation process. 

2. Applications shall be submitted electronically using MS Word v. 98 or later to the email listed on the awards nomination form by 11:59 p.m. (CST) March 31. It is the applicant’s responsibility to contact central office if an email confirmation for receipt of nomination materials is not received. Every effort will be made to award the scholarship by June 1.

ADAA Foundation

KAY MOSLEY DISTINGUISHED SERVICE AWARD (MS-Word Form)

This relatively new award comes from the Kay Mosley Fund and was created in honor of ADAA’s Past President Kay Mosley, CDA, of Oklahoma City, Okla. Ms. Mosley was President of the ADAA in 2003-04 and continues to be active in state and local dental assisting affairs and with the ADAA Foundation.

The Mosley Fund has been incorporated into the Foundation and the award specifically recognizes significant contributions to the ADAA and to the Foundation itself and to the field of dental assisting and dentistry. These contributions shall have been demonstrated through leadership, personal accomplishment and vision for the profession of dental assisting.

A $2,500 honorarium is presented with this award along with commemorative materials. Fee for gala tickets for recipient and a guest shall be waived.

 

Qualifications
The award is funded by the Kay Mosley Fund and the recipient is selected by the ADAA, ADAAF and a representative of the Fund. It does not carry a mandate for annual presentation.
1.   The recipient must be a current member of ADAA.
2.   Recipient must attend the Annual Conference to receive the award (unless the winner is on active military duty).
3.   No ADAA board member, officer, trustee or staff member may be considered.
4.   No one may receive the award more than once.
5.   Shall have made significant contributions to the ADAA and ADAAF and the profession of dentistry in general.

 

Procedure
Any member of the ADAA or the ADAA Foundation may nominate another using this ADAAF Award Nomination Form and submitting it by June 30 to the Awards Chair at Central Office. A letter of recommendation is suggested.
